Resident Advisor Duty Phone Agreement

I, _____________________________________, agree to make restitution for any loss or damage incurred while the Duty phone is checked out to me. If I do not properly log the phone in or out, I may have to pay restitution for damages that I did not cause. Additionally, I understand that I must sign the phone out and back in on the appropriate "Duty Phone Checkout Sheet." If I find the phone to be missing or not in proper working order, I will notify the Residence Life supervisor immediately. Furthermore, I understand that the phone may not be used for incoming or outgoing personal phone calls, and is only to be used for emergencies.

I understand that the cost for replacement of each phone if lost or broken is $160.
